Allah says in the Quran that prayer is something that Allah has apportioned and timed and it is obligatory to pray at its proper time. So the default is that you really need to work around the prayer timings and if you have an exam for four hours, five hours, I mean, they must allow a bathroom break, go speak to the instructor, speak to the professor, try your best. Don't just give up and say okay, I have an exam. Try your best to see if you can pray and most people will be accommodating in this regard. If supposing it is a one off exam that truly there is absolutely no time. Some of the scholars have said you may combine the head and outside and you'll be combined

But before you get there, make sure you have tried your best and gotten tried to get permission and see if you can pray the proper prayers on time.
